📋 Description

• After fully trained (90 day training period) and meeting expectations, this position is 100% work from home. • Medication Therapy Management (MTM) pharmacists with advanced training, Board Certifications or similar credentials to work in a progressive area where pharmacists provide MTM services directly to patients. • The pharmacist provides one-on-one phone/video based consultations with commercial and Medicare Part D plan members. • The pharmacist may be responsible for assisting with the intake/enrollment process. • The pharmacist will consult with the patient to identify, resolve and prevent medication related problems. • The pharmacist assesses each of the patient’s medications for indication, effectiveness, and safety and adherence issues and incorporates lab data in the assessment process, when available. • The pharmacist will prepare a medication summary and personal care plan with recommendations for the patient to share with their medical providers. • The pharmacist will follow up with the patient to monitor the patient’s drug therapy and to ensure positive outcomes as appropriate.

🎯 Requirements

• Arizona Pharmacist License in good standing • Must reside in the State of Arizona • Excellent verbal and written communication skills • Well organized with the ability to effectively prioritize work • Customer focused • Ownership and results driven • Attention to detail • Proficient use Microsoft Office applications, e.g. Outlook, Excel, Word • Ability to effectively work within a team • Flexible in adapting to change • Ability to effectively plan, prioritize, organize time and workload • Skilled in identifying and solving issues and recommending process improvements • Ability to work successfully in a deadline-oriented environment • Pharm D Degree
