📋 Description

• Deliver patient care services through a remote technology platform • Independently provide holistic, evidence-based care for patients aged 18 months and above • Assess, diagnose, treat, and manage health problems • Provide health counseling and disposition planning • Provide education and treatment for pregnancy prevention, STI prevention, and safer sex practices • Evaluate primary care, acute, chronically ill, and transitional care patients • Provide contraceptive care counseling and medication management • Educate patients on health maintenance and respond to patient care inquiries • Document all patient care in an EHR according to MinuteClinic policies and procedures • Coordinate patient care with internal and external colleagues, including the broader patient-centered medical home • Work within a patient care team including Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice

🎯 Requirements

• Currently licensed in Tennessee • Minimum of two years of medically-relevant experience or equivalent • Completion of a Master’s Degree level Family Nurse Practitioner program • Current National Board Certification • State of Employment license to practice in the Advanced Practice Nurse role • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multi-task • Initiative, problem solving ability, adaptability and flexibility • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Capacity to collaborate with professional colleagues • Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required • Broadband connectivity and a quiet setting with a neutral background for remote care delivery • Ability to uphold patient privacy per CVSH guidelines • Ability to hardwire into the internet connection preferred • Must live within 3 driving hours to Nashville or Knoxville, Tennessee • Willingness and ability to obtain additional licensure as needed • Ability to work overnight, 32 hours per week, including every other Saturday and Sunday, from 9:30 p.m. to 6:00 a.m.
