📋 Description

• Deliver patient care services through a remote technology platform • Independently provide holistic, evidence-based care, including assessment, diagnosis, treatment, health problem management, health counseling, and disposition planning • Provide education and treatment for pregnancy prevention, STI prevention, and safer sex practices • Evaluate primary care, acute, chronically ill, and transitional care patients • Provide patient counseling, contraceptive care counseling, and medication management • Educate patients on health maintenance and respond to patient care inquiries • Document patient care in an EHR according to MinuteClinic policies and procedures • Coordinate patient care with internal and external colleagues, including the broader patient-centered medical home • Work within a patient care team alongside Providers, Collaborative Physicians, paraprofessionals, Pharmacists, and other healthcare team members • Work independently, prioritize and solve problems, take initiative, and advocate for patients and the practice

🎯 Requirements

• Current, unrestricted license in one of CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV • Minimum of two years of medically-relevant experience or equivalent • Completion of a Master’s Degree level Family Nurse Practitioner program • Current National Board Certification • State of Employment license to practice in the Advanced Practice Nurse role • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multi-task • Initiative, problem solving ability, adaptability and flexibility • Ability to work without direct supervision and practice autonomously • Proficient with information management and technology • Capacity to collaborate with professional colleagues • Depending on the market, proficiency in speaking and writing additional languages, including Spanish, may be required • Broadband connectivity • Quiet setting with a neutral background for remote visits • Ability to uphold patient privacy per CVSH guidelines • Ability to view and read a computer screen and other electronic devices • Ability to work a full-time weekend evening schedule: three weekdays and every Saturday and Sunday, 2:30 p.m. to 11:00 p.m. • If hired, willingness and ability to obtain additional licensure as needed
