📋 Description

• Conduct approximately 30-minute online Video Visits with health plan members • Review medications and medical histories • Educate members about their health and answer questions • Help close gaps in care for Medicare Advantage, Medicaid, and certain commercial plan members • Provide Video Visit telehealth services only • Work in a private space with adequate internet connection • Visit people nationwide and connect them to appropriate care • Attend regular manager, team, and clinical meetings • Meet Signify Health operational, clinical, and member experience KPIs and metrics • Maintain required training, affiliations, credentials, and compliance

🎯 Requirements

• Full-time, board-certified nurse practitioner or physician assistant • Active state licenses in coverage areas • FNP or AGNP certification preferred for NPs • Maintain board certification • Able to obtain and maintain multi-state licenses in coverage area, as required • Provide 40 hours of availability weekly • Consistently provide 320+ capacity units per month • Consistently complete 220+ visits per month • Maintain training, affiliation, and compliance with Video Visit requirements • Trained and credentialed for all health plan clients and lines of business in primary coverage state(s) • Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format preferred • Broadband connectivity with video and audio enabled • Quiet setting with neutral background for conducting visits • Ability to uphold health plan member privacy per CVS Health guidelines • Effective verbal, written, and electronic communication skills • Ability to work without direct supervision and practice autonomously • Proficiency with information management and technology • Ability to interact remotely with members, sit, write, operate computer and phone, speak intelligibly, and hear member responses • Ability to view and read computer screens, iPad, and other electronic devices
