• Supervises CHWs to include work allocation, training and problem resolution; evaluates performance and makes recommendations for personal actions; motivates employees to achieve peak productivity and performance. • Available for daily contact with CHWs in assigned region to provide resource navigation and peer support. • Document CHW program workflow to inform policies and procedures. • Assist CHWs with maintaining continuing education and certification. • Lead team huddles and other meetings as assigned. • Carry a 50% to 75% caseload • Provide troubleshooting support to CHWS • Work with internal care management team to facilitate care coordination, case management, resource delivery, and system navigation to hard-to-reach populations.
• High School Diploma or General Education Diploma (GED) is required. • Ability to lead and train employees, to include organizing, prioritizing and scheduling work assignments. • Minimum of two years leading a team of 2 to 4 employees • Minimum of two (2) years of experience in either volunteer or paid position working in community setting with at risk populations providing coordination of services is preferred • Ability to attain Community Health Worker certification within 1 year of hire • Bilingual (Spanish) a plus • Previous experience servicing diverse populations in rural areas is a plus • Must possess/maintain a valid driver’s license, current automobile insurance and a driving record that meets the guidelines and requirement of the organization.
