📋 Description

• Provide high-quality, patient-centered care through telephone triage and clinical assessment • Perform telephone triage using established protocols and nursing judgment • Assess symptoms, prioritize patient needs, and determine appropriate care or follow-up • Provide patient education, guidance, and reassurance • Document patient encounters in Epic EMR in real time • Escalate urgent or emergent situations according to policies • Communicate with on-call providers, clinic teams, and patients to ensure continuity of care • Utilize the nursing process to assess, diagnose, plan, implement, and evaluate individualized care • Monitor and evaluate practice to ensure safe patient care • Contribute to research, quality improvement, and evidence-based practice • Mentor and precept colleagues, students, and healthcare team members • Perform other duties as required or assigned

🎯 Requirements

• Active, unrestricted RN license; multi-state compact required • Minimum 2–3 years of clinical experience with a strong background in triage, ER, Med-Surg, and/or Primary Care • Proficiency in Epic EMR and remote communication systems • Excellent assessment, critical thinking, and decision-making skills • Strong communication, documentation, and time management abilities • Self-motivated and comfortable working independently in a remote environment • Associate degree required; BSN preferred • Graduate from an accredited Nursing Program required • Basic Life Support (BLS) Certificate required upon hire • Licensed Registered Nurse with NH eligibility • Reliable internet and a secure HIPAA-compliant workspace, free from distractions • Eligible candidates must reside in one of the listed compact eligible states
