📋 Description

• Coordinate and facilitate quality, cost-effective care for ESRD and CKD patients • Minimize fragmentation of the healthcare delivery system • Function autonomously and collaborate with the healthcare team • Coordinate care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager • Support ESKD and CKD patients throughout California, Oregon, Nevada, and Washington as needed • Travel throughout the assigned territory • Accommodate patient needs and nephrology partner availability on a Monday–Friday schedule

🎯 Requirements

• A.D.N degree from an accredited school of nursing required; a three-year diploma from an accredited diploma program may be substituted for the nursing degree • B.S.N preferred • Current Registered Nurse (RN) license in the state of practice required, with licensure in California, Oregon, and Washington required or willingness to obtain missing licensure • Continuing education credits maintained as required by state of practice • Case management experience required; alternatively, 3 or more years assessing patient needs, developing and managing individualized care plans, coordinating interdisciplinary services, and supporting complex healthcare transitions • Current CPR certification required • Previous healthcare performance coaching experience required • Demonstrated knowledge of data and management of clinical, financial, and patient satisfaction outcomes • Demonstrated experience and effectiveness as a change agent • Demonstrated knowledge of CQI techniques • Ability to adapt to new/electronic processes and increased productivity expectations for Capella implementation • Current driver's license in the state employed, positive driving record, and ability to meet insurance coverage requirements • Basic computer skills and proficiency in MS Word and Outlook • Functional proficiency with DaVita clinical software programs, including Capella, within 90 days of employment • Home office with internet connectivity of at least 1MB upload and 1MB download speed • Minimum of five years' clinical nursing experience strongly preferred • Minimum of three years' renal nursing experience preferred • Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
