📋 Description

• Assist complex patient population in navigating the health care system • Support and look at a patient holistically • Patient engagement • Patient connection • Case Management • Functions autonomously and in collaboration with healthcare team members • Coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for Chronic Kidney Disease (CKD) patients or ESKD (End-Stage Kidney Care) patients • Provide coordination of care between patient, family, physician, provider, and care teams

🎯 Requirements

• Degree from accredited school of nursing required • B.S.N preferred • Current Registered Nurse (RN) license in the state of practice required • Continuing education credits maintained as required by state of practice required • Minimum of five (5) years’ of RN experience in clinical nursing Required • Previous Case Management experience required • Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes • Experience and passion for educating patients • Current CPR certification required • Ability to adapt to new/changing technology • Current driver’s license in state employed with positive driving record and able to meet requirements of insurance coverage required • Basic computer skills and proficiency in MS Word and Outlook required
