📋 Description

• Deliver specialized diabetes care to members through a next-generation virtual diabetes clinic • Conduct focused and thorough virtual patient assessments, order advanced diagnostics, interpret laboratory and imaging data, and develop treatment plans • Formulate diagnoses and individualized treatment plans, including medication management, lifestyle modifications, and monitoring recommendations • Collaborate with PCPs, endocrinologists, pharmacists, clinical nurses, social workers, certified diabetes educators, family members, and caregivers • Serve as clinical advisor and provide clinical escalation support to specialty clinic staff and other teams during business hours • Participate in regular panel review discussions and advise on medical management • Perform comprehensive disease, medication, quality-of-care, social, home-health, and DME assessments • Use and provide feedback on the company’s electronic health information system • Maintain accurate and current patient records in compliance with legal and ethical guidelines • Participate in quality improvement initiatives and ongoing professional development • Adhere to applicable laws, regulations, industry standards, patient privacy requirements, and telehealth regulations

🎯 Requirements

• Nurse practitioner or physician assistant with 5 or more years working in outpatient clinical practice, ideally in an endocrinology or diabetes-focused clinic • Minimum of 2 years of experience concentrated in diabetes management and education required • Proficiency in using telehealth technology and electronic health records (EHR) • Virtual care experience is preferred • Understanding of managed care is a plus, including STARS/HEDIS measures, coding clinical comorbidities, and identifying clinical care gaps • Proficiency in English required; Spanish ideally • Multi-state licensure required, with willingness to obtain and maintain additional licensure as requested • Master's or Doctoral degree • Active and clear RN and APRN license, or PA licensure required at time of hire and maintained while employed • Active BLS required at time of hire and maintained while employed • Experience with diabetes management, treatment modalities, medication management, and lifestyle modification • Ability to use electronic health information systems • Compliance with patient privacy and telehealth regulations
