• Provide clinical oversight to a patient panel across the country with an interdisciplinary team • Deliver care to complex patients across multiple geographies using video telehealth visits and a multidisciplinary team • Work with patients, caregivers, and community resources to improve health literacy, self-efficacy, and patient-centered outcomes • Develop and standardize team-based community practice with clinical leadership • Lead a multidisciplinary team • Coordinate with patients’ caregivers/family, non-Devoted providers, and Devoted clinical and psychosocial programs • Collaborate with PCPs, specialists, pharmacy, disease management and education, social work, and palliative teams • Use the Orinoco electronic health information system for visits and provide interface improvement feedback • Promote a patient-centric, open clinical culture among patients and colleagues
• Fellowship-trained physician • Board-certified physician • At least 3 years of clinical experience • Geriatric background and experience caring for older adults with complex chronic conditions • Experience managing other clinicians • Telehealth experience preferred • Multi-state licensure preferred or willingness to obtain additional licensure as requested • Must be licensed to practice medicine by the [MARKET] Medical Board • Organized with meticulous attention to detail • Ability to provide targeted and accurate feedback • Ability to work collaboratively in a fast-paced, changing, startup-minded organization
