📋 Description

• Reaching out to patients while inpatient to introduce TOC program and communicate post discharge follow up from TOC clinicians • Scheduling post discharge follow ups (PCP/Specialty) • Reaching out to hospitals and provider offices to obtain clinical documentation • Submitting Home Care and DME orders as well as vendor follow up • Monitoring return fax queue and researching provider profile for updated contact and fax information • Provide additional operational support for assigned team and manager as designated • Ensure patients are prepared for telehealth visits by assisting with technology prep • Collaborate internally in clinical rounds to discuss patients at risk for readmission • Coordinate lab testing and imaging, and tracking results to ensure that they are reviewed by the ordering clinician

🎯 Requirements

• 2+ years of patient facing and care coordination experience within healthcare, managed care, home health, and/or community health setting. • Experience working in a telemedicine environment. • Managed Care / Health Plan experience • MA Certification (desired), expired MA certifications are OK! • Thrive in a fast-paced, high-energy, and rapidly growing organization • Skilled at easing stress and anxiety for patients during difficult times • Strong communicator, able to effectively interact with various stakeholders including doctors, pharmacies, nurses, and caregivers • Comfortable working with patients via video, phone, portal • Entrepreneurial self-starter who takes initiative and thrives as a collaborative team player • Quick to adopt and learn new technologies, including electronic health records and various computer platforms (e.g., Google Chrome, Google Sheets) • Committed to delivering healthcare with the same quality and care you'd want for your own family • Passionate about helping others improve their health and navigate the healthcare system with compassion, empathy, and warmth • Highly organized and adaptable, able to manage a large member panel amidst frequent changes.
