📋 Description

• Conduct telehealth visits for patients enrolled in the House Calls program • Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patient requires primary care services • Perform care management needs assessments, follow-ups, and care plan adjustments • Review patient histories, labs, imaging, and diagnostic results • Support medication management and reconciliation • Coordinate care with primary care providers, specialists, and care management teams • Document visits and clinical decisions accurately in the EMR • Ensure compliance with clinical protocols, quality standards, and regulatory requirements • Participate in interdisciplinary case reviews and quality improvement initiatives

🎯 Requirements

• Active Nurse Practitioner (NP) license in the required state(s) • National NP certification (ANCC or AANP) • Minimum 2+ years of clinical experience as a Nurse Practitioner • Experience in house calls, home health, geriatrics, primary care, or internal medicine strongly preferred • Prior telehealth or remote care experience preferred • Strong clinical judgment and comfort managing complex patients remotely • Excellent communication and documentation skills • Must be licensed in NY and one of the other states listed or willing to obtain • Proficiency with EMR/EHR systems • Comfortable using telehealth platforms and remote monitoring tools • Active DEA registration (or willingness to obtain, if applicable)
