📋 Description

• Collaborate with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care • Provide clinical and non-clinical support to patients • Provide urgent healthcare via telephone and tele-video modalities • Develop and implement clinical plans of care for adult patients with chronic and complex conditions • Gather patient histories, perform physical examinations, and obtain diagnostics as needed • Develop and implement treatment plans based on patient goals and current conditions • Identify and close gaps in care • Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, adherence, and crisis prevention • Coordinate care and resources with clinical team members and medical providers • Maintain patient medical records and documentation • Participate in continuing education • Prescribe medication as permitted by state prescribing authority

🎯 Requirements

• Licensed Nurse Practitioner • MS in Nursing • Active national NP certification • Valid, current, active, and unrestricted Family or Adult Nurse Practitioner license in Ohio, New Jersey, and/or Massachusetts • Valid, current, active RN Compact License • 2+ years of experience managing complex care cases • Experience working with Electronic Medical Records (EMR) • DEA registration or eligibility preferred • Active Medicaid number in Ohio, New Jersey, and/or Massachusetts highly preferred • Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred • Candidates must reside within reasonable commuting distance of the posting location(s), unless an accommodation is granted • New candidates in certain patient/member-facing roles must become vaccinated against COVID-19 and Influenza, unless an acceptable explanation is provided
