📋 Description

• Collaborate with company physicians, patients’ other physicians and providers, and family members to develop complex plans of care • Provide clinical and non-clinical support to patients • Provide urgent health care via telephone and tele-video to patients receiving home and community-based services • Develop and implement clinical plans of care for adult patients with chronic and complex conditions • Gather patient history, physical examinations, and diagnostics as needed • Develop and implement treatment plans based on patient goals and current conditions • Identify and close gaps in care • Address patients’ and families’ physical and psychosocial needs with interdisciplinary team support • Educate patients and families about medication usage, side effects, illness progression, diet and nutrition, adherence, and crisis prevention • Coordinate care and resources with clinical team members and medical providers • Maintain patient medical records and medical documentation • Participate in continuing education as required • Prescribe medication as permitted by state prescribing authority • Work Monday–Friday, 8:00 a.m.–5:00 p.m. EST, with rotating on-call coverage 2–3 days per month

🎯 Requirements

• Master of Science in Nursing (MS) • Active national Nurse Practitioner (NP) certification • Valid, current, active, unrestricted Family or Adult Nurse Practitioner license in Ohio and/or New Jersey • Valid, current, active RN Compact License • 2+ years of experience managing complex care cases • Experience working with Electronic Medical Records (EMR) • Possession of DEA registration or eligibility preferred • Active Medicaid number in Ohio and/or New Jersey highly preferred • Experience managing complex care cases for developmental disabilities and chronically ill patients strongly preferred • Must work Monday–Friday, 8:00 a.m.–5:00 p.m. EST with rotating on-call coverage • Candidates must reside within reasonable commuting distance from an office unless an accommodation is granted • New candidates in certain patient/member-facing roles must be vaccinated against COVID-19 and Influenza, unless an acceptable explanation is provided
