📋 Description

• Support disease management programs and physicians’ care plans through drug therapy evaluation and consultation for program participants • Evaluate members’ prescription needs and therapies by reviewing medication histories to determine appropriateness • Assess member compliance with plans through direct contact with members and physicians • Determine potential barriers to compliance and identify solutions • Ensure members understand the appropriate use of current prescriptions • Review medications with physicians when therapies do not meet national standards • Counsel physicians regarding alternative medications • Serve as a resource to the clinical team in disease management and other clinical areas

🎯 Requirements

• BA/BS in Pharmacy and a minimum of 2 years of clinical experience, or an equivalent combination of education and experience • Current unrestricted Pharmacist license in applicable state(s) required • PharmD preferred • Call center experience strongly preferred • Computer skills • Ability to work Monday–Friday, 12:00 PM–8:30 PM EST after training • Must reside within a reasonable commuting distance of a posting location unless an accommodation is granted as required by law
