📋 Description

• Provide skills-based individual and group sessions to patients and caregivers in treatment at Equip. • Implement motivational strategies with patients and caregivers to engage with their care plans. • Participate in multidisciplinary treatment team huddles to support patient progress in treatment. • Actively listen and guide patients and caregivers to anticipate challenges. • Maintain an availability calendar allowing patients and caregivers to schedule appointments. • Conduct proactive outreach, missed appointment recovery, reminders, tracking, and support. • Engage in problem-solving to ensure care plan adherence. • Ensure personalization of care plans by making program recommendations and coordinating services. • Build a relationship of respect and trust with patients. • Complete timely documentation of caregiver and patient interactions. • Attend and participate in internal meetings as well as initial and ongoing training as assigned.

🎯 Requirements

• Lived experience with eating disorder recovery with ability to identify as being in a solid space of recovery. • 2+ years of experience working directly with patients in a setting that utilizes protocolized care. • Proven comfort working within structured care models or evidence-based protocols. • Proven ability to balance structured protocols with personalization. • A proven track record of taking accountability for patient care plans. • Experience guiding patients and families in achievement of treatment goals. • Ability to connect with a range of diverse people with varying lived experiences. • Passion for evidence-based care and ability to learn, practice, and develop new coaching skills. • Strong organizational and time management skills with a proven track record of meeting goals. • Comfortable working remotely (including the requirement to be on camera). • Ability to commit to working two evenings until 8pm of your choice per week.
