📋 Description

• Monitor physician EHR inboxes and patient portal requests • Perform tasks within LPN scope of practice and respond to patient inquiries according to established protocols • Refill prescription requests according to established protocols • Notify providers and complete patient-care tasks for critical results • Coordinate post-discharge and post-operative follow-up care • Facilitate referrals and assist with prior authorization requests requiring clinical consultation • Respond to requests from hospitals, Public Health departments, nursing homes, funeral homes, and Departments of Labor or Motor Vehicles • Manage Remote Patient Monitoring (RPM) results and track real-time disease results • Provide education on managing chronic diseases including diabetes, hypertension, and COPD • Medically screen and qualify patients for clinical trials • Conduct patient follow-ups and education • Counsel patients with chronic diseases • Follow up with patients between provider visits • Refill medications per protocol • Facilitate communication among healthcare professionals and patients to improve outcomes and relieve provider stress • Carry out additional responsibilities to support the healthcare team and enhance patient education

🎯 Requirements

• Licensed Practical Nurse (LPN) • 5+ years of full-time clinical experience as an LPN • 2 years of primary care experience • Primary care experience with adult, pediatric, and geriatric patients • Experience using at least two electronic health record (EHR) systems • Active, unencumbered e-NCL or Enhanced Nurse Licensure Compact license • Willingness to obtain additional licenses at the company's request • Additional non-compact state licensure considered an asset • Ability to work remotely at home in a private HIPAA-compliant workspace • Ability to house company equipment • Broadband internet access • Ability to participate in telephone or video staff meetings • Ability to sit for extended periods, talk, and actively listen • Ability to hear in the normal range and wear a headset/earpiece • Good visual acuity for reading computer screens, scripts, and forms • Communication, customer service, teamwork, multitasking, EHR, data entry, and computer skills • Ability to maintain professional relationships with diverse individuals • Flexibility and adaptability to change and stress management • May require occasional overtime or adjusted starting times
