📋 Description

• Monitor physician EHR inboxes and patient portal requests • Perform tasks within the LPN scope of practice and respond to patient inquiries according to established protocols • Process prescription refill requests, notify providers, and complete patient-care tasks for critical results • Coordinate follow-up care after discharge or surgery • Facilitate referrals and collaborate on prior authorization requests requiring clinical consultation • Assist with requests from hospitals, public health departments, nursing homes, funeral homes, and Departments of Labor or Motor Vehicles • Manage Remote Patient Monitoring (RPM) results by tracking real-time disease results • Provide patient education on chronic disease management, including diabetes and hypertension • Counsel patients with chronic diseases and follow up between provider visits • Medically screen and qualify patients for clinical trials and perform related follow-up and patient-care activities • Carry out additional responsibilities to assist the healthcare team and enhance patient care

🎯 Requirements

• Licensed Practical Nurse certificate • Minimum of 2–4 years of full-time clinical experience as an LPN, particularly in primary care for adult, pediatric, and geriatric patients • Experience using at least two electronic health record (EHR) systems • Active, unencumbered LPN license with multistate privileges under the Enhanced Nurse Licensure Compact (eNLC) • Willingness to obtain additional licenses at the company's request • Strong communication, teamwork, customer service, empathy, patience, active listening, attention to detail, organization, and time-management skills • Ability to sit for extended periods, talk, and engage in active listening without visual contact with patients • Ability to hear in the normal range and wear a headset/earpiece • Good visual acuity to read computer screens, scripts, and forms • Ability to work remotely at home in a private HIPAA-compliant environment • Ability to house company equipment needed to perform the job • Broadband internet • Ability to thrive in a fast-paced environment and adapt to change • May require occasional overtime or adjusted start times
