📋 Description

• Track and respond to inbound and outbound calls for a multi-state nurse triage telephone service • Use Schmitt-Thompson telephone triage protocols • Document patient interactions in clients’ EHR and/or Call 4 Health platform • Provide clinical assessment and triage patients by phone or patient portal • Process medication prescription refills per established protocols • Notify providers of critical results • Coordinate post-discharge and post-operative follow-up care • Facilitate referrals and assist with prior authorization requests requiring clinical consultation • Assist with requests from hospitals, nursing homes, funeral homes, and Departments of Labor or Motor Vehicles • Participate in Remote Patient Monitoring initiatives by tracking vital signs • Educate patients on chronic disease management, including diabetes, hypertension, and COPD • Screen and qualify patients for clinical trials • Conduct follow-ups and patient education • Support healthcare teams and enhance patient care delivery

🎯 Requirements

• Active Compact State RN License plus multiple state licenses • Additional active licenses in CA, NV, OR, AK, CT, and MI are required • Active IL, MA, NY, MN, and DC licenses highly preferred • Bachelor’s or Associate’s Degree in Nursing • Minimum of 2-4 years of full-time clinical experience as a Registered Nurse • Previous telephone triage experience using electronic triage systems • At least 1 year of remote work experience • At least 1 year of telephone triage experience • At least 1 year of experience with multiple EHR/EMR systems • At least 1 year of remote triage experience • Ability to work days, nights, weekends, and rotating schedules • Open availability and flexibility based on business needs • Reliable phone triage, multitasking, electronic health records, critical thinking, computer, and clinical expertise • Ability to sit for extended periods, talk, and engage in active listening • Ability to hear in normal range and wear a headset/earpiece • Good visual acuity to read computer screens, scripts, and forms • Ability to work remotely from a private HIPAA-compliant workspace • Ability to house company equipment • Broadband Internet access • Ability to participate in staff meetings via telephone or video • Ability to thrive in a fast-paced environment and manage stress effectively
