📋 Description

• Provide high-quality behavioral health care within a supported care model • Conduct therapy sessions independently • Collaborate with care teams for effective patient treatment • Utilize telehealth platforms for therapy and clinical documentation

🎯 Requirements

• Active Licensed Clinical Social Worker (LCSW) license or Licensed Professional Counselor (LPC/LMHC) • 3+ years of clinical experience (community mental health, integrated care, or similar settings preferred) • Comfort providing therapy independently within a supported care model • Strong clinical judgment in assessment, diagnosis, and treatment planning • Clear, professional communication and collaboration with care teams • Comfort using telehealth platforms and clinical documentation tools • Commitment to full-time work and continuity of care
