📋 Description

• Provide therapy independently within a supported care model • Conduct assessment, diagnosis, and treatment planning • Collaborate with care teams • Use telehealth platforms and clinical documentation tools • Support continuity of care • Work as a full-time clinician partnering with community health organizations to expand access to behavioral health care

🎯 Requirements

• Active Licensed Clinical Social Worker (LCSW) license; must have an active and unrestricted license in Michigan • 3+ years of clinical experience (community mental health, integrated care, or similar settings preferred) • Comfort providing therapy independently within a supported care model • Strong clinical judgment in assessment, diagnosis, and treatment planning • Clear, professional communication and collaboration with care teams • Comfort using telehealth platforms and clinical documentation tools • Commitment to full-time work and continuity of care
