Utilizing the Form Health app, patients can engage in frequent video visits, message their Care Team, and monitor their progress. FORM partners with employers, health plans, and health providers to offer comprehensive obesity care. Their services are covered by national health insurance plans, including Medicare, focusing on a healthier future for their clients.

📋 Description

• Provide direct patient care in a telemedicine setting for obesity and comprehensive diabetes management • Manage insulin titration and patients using insulin pumps and continuous glucose monitors • Maintain an individual patient panel or collaborate with and supervise APPs and Registered Dietitians • Review charts and conduct initial and return visits • Manage CKD/ESRD and CAD/CV conditions • Provide advanced treatment planning and medical management for complex endocrinology cases • Prescribe non-controlled obesity and diabetes medications • Ensure consistent clinical documentation, protocols, and treatment pathways • Support colleagues’ professional development through feedback and continuing education • Participate in team-based care initiatives, departmental meetings, and quality improvement efforts

🎯 Requirements

• MD or DO degree • Valid and current Board certification in Endocrinology by the American Board of Internal Medicine (ABIM) • 2 years of clinical experience and pharmacotherapy in obesity medicine or endocrinology, post residency, OR completed an accredited Fellowship program in Endocrinology or Obesity Medicine in lieu of years of experience • Active and unrestricted medical state license in any US state • Eligibility to obtain additional state licenses; IMLC preferred but not required • Currently opted-in with Medicare • Competency managing complex co-morbidities, including CKD, ESRD, and CAD/CV conditions • Expertise in advanced diabetes management, including CGMs, insulin pumps, and complex insulin regimens • Experience working with APPs, medical residents, or trainees in a clinical academic setting • Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity • Open to W-2 roles of 32, 36, or 40 hours per week • Commitment to patient-centered, culturally competent care • Excellent communication and interpersonal skills • Bilingual candidates with medical fluency in Spanish desired but not required • Synchronous telehealth experience preferred
