Utilizing the Form Health app, patients can engage in frequent video visits, message their Care Team, and monitor their progress. FORM partners with employers, health plans, and health providers to offer comprehensive obesity care. Their services are covered by national health insurance plans, including Medicare, focusing on a healthier future for their clients.

📋 Description

• Provide direct patient care via telemedicine for obesity and comprehensive diabetes management, including insulin titration and patients using insulin pumps or CGMs • Manage an individual patient panel or collaborate with and supervise Endocrine APPs and diabetes-specializing Registered Dietitians as needed • Review charts and conduct initial and return visits • Manage CKD/ESRD and CAD/CV conditions and complex endocrinology cases • Provide advanced treatment planning and medical management support for escalated concerns and specialized protocols • Prescribe non-controlled obesity and diabetes medications • Ensure consistent clinical documentation, protocols, and treatment pathways • Foster colleagues’ clinical growth, provide feedback, and support continuing education • Participate in team-based care initiatives, departmental meetings, and quality improvement efforts

🎯 Requirements

• MD or DO degree • Valid and current Board certification in Endocrinology by the American Board of Internal Medicine (ABIM) • 2 years of clinical experience and pharmacotherapy in obesity medicine or endocrinology, post residency, OR completed an accredited Fellowship program in Endocrinology or Obesity Medicine in lieu of years of experience • Active and unrestricted medical state license in any US state, with eligibility to obtain additional state licenses; IMLC preferred but not required • Currently opted-in with Medicare • Competency managing complex co-morbidities, including CKD, ESRD, and CAD/CV conditions • Expertise in advanced diabetes management, including CGMs, insulin pumps, and complex insulin regimens • Experience working with APPs, medical residents, or trainees in a clinical academic setting • Adherence to evidence-based treatments, behavioral interventions, nutrition, and pharmacotherapy for endocrine disorders and obesity • Willingness to work a W-2 role of 32, 36, or 40 hours per week • Excellent communication and interpersonal skills • Synchronous telehealth experience preferred • Spanish medical fluency desired but not required • ABOM certification highly preferred
