📋 Description

• Supports the GCHP mission to improve health through quality care and services • Collaborate with internal partners including social workers, health education, pharmacy staff, and member services • Assess and identify members who may benefit from additional education and clinical support • Promote cost-effective healthcare delivery by coordinating with the medical management team • Engage members into care coordination activities to reduce care gaps • Formulate member-centric care plans in partnership with members • Provide member advocacy and education to maintain or improve clinical outcomes • Prepare written reports and ensure confidentiality of information

🎯 Requirements

• Bachelor's Degree in a health-related field preferred • Registered Nurse (RN) valid and current license • 2+ years of experience in the health care field • Care management, discharge planning, or equivalent combination of education and experience • Managed care experience in a health plan preferred • Ability to evaluate clinical and other health data and to communicate effectively both orally and in writing • Strong organizational skills • Critical thinking skills • Flexibility in role delineation and workload assignments • Bilingual in English and Spanish preferred
