📋 Description

• Conduct Medicare Annual Wellness Visits via telehealth in accordance with CMS guidelines • Virtually review and update patient medical histories, medication lists, immunization records, and family history • Assess patients' health status, vital signs when available, cognitive function, and chronic disease risk factors using remote monitoring tools when applicable • Provide virtual patient education on preventive measures, lifestyle changes, and community resources • Develop and update personalized prevention plans based on patient needs and physician guidance • Collaborate with the supervising physician on documentation and follow-up of identified health concerns using EHR systems • Identify HCC codes requiring recapture reporting to CMS • Maintain accurate patient records in compliance with HIPAA and organizational standards • Facilitate virtual referrals to healthcare services and specialists • Monitor patient progress and adherence to prevention plans through telehealth follow-ups, providing support and adjustments • Stay current on Medicare guidelines, telehealth best practices, preventive care, and chronic disease management • Participate in virtual team meetings, training, and continuing education

🎯 Requirements

• Must be fully bilingual in Korean and English • Must be located in GA • Current and unrestricted Registered Nurse (RN) license in the state of GA • Availability for PRN work Monday through Friday, 8:00am to 5:00pm, approximately 20 hours per week • Minimum of 3 years of patient care nursing experience • Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms highly desirable • Strong knowledge of Medicare guidelines, telehealth practices, and preventive care strategies preferred • Excellent interpersonal and communication skills, with the ability to educate and engage patients effectively in a virtual environment • Proficient in electronic health records (EHR) systems, telehealth technology, and remote medical documentation • Ability to work independently and manage time effectively in a remote work setting • Strong organizational skills and attention to detail • Must provide an internet connection capable of video calls and system access, with minimum speeds of 100 Mbps download and 10 Mbps upload • Must comply with security policies and procedures protecting PHI, PII, and company intellectual property
