📋 Description

• Conduct Medicare Annual Wellness Visits via telehealth in accordance with CMS guidelines • Complete required patient assessments and screenings • Virtually review and update medical histories, medication lists, immunization records, and family histories • Assess current health status, cognitive function, vital signs when available, and chronic disease risk factors • Use remote monitoring tools when applicable • Provide virtual patient education on preventive measures, lifestyle changes, and community resources • Develop and update personalized prevention plans with physician guidance • Collaborate with the supervising physician on documentation and follow-up • Identify HCC codes requiring recapture reporting to CMS • Maintain accurate patient records in compliance with HIPAA and organizational standards • Facilitate virtual referrals to healthcare services and specialists • Monitor patient progress and adherence to prevention plans through telehealth follow-ups • Stay current on Medicare guidelines, telehealth best practices, preventive care, and chronic disease management • Participate in virtual team meetings, training, and continuing education

🎯 Requirements

• Fully bilingual in Korean and English • Must be located in GA • Current and unrestricted Registered Nurse (RN) license in the state of GA • Availability for PRN work Monday through Friday, 8:00am to 5:00pm, approximately 20 hours per week • Minimum of 3 years of patient care nursing experience • Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms highly desirable • Knowledge of Medicare guidelines, telehealth practices, and preventive care strategies preferred • Excellent interpersonal and communication skills • Proficiency in electronic health records (EHR) systems, telehealth technology, and remote medical documentation • Ability to work independently and manage time effectively in a remote work setting • Strong organizational skills and attention to detail • Ability to provide own internet connection with minimum 100 Mbps download and 10 Mbps upload • Must comply with HIPAA, organizational security policies, and patient/company data protection requirements • Must be accessible during stated working hours • Must participate in virtual meetings with cameras on
