📋 Description

• Conduct Medicare Annual Wellness Visits via telehealth under direct physician supervision and in accordance with CMS guidelines • Review and update patient medical histories, medication lists, immunization records, and family histories virtually • Assess health status, vital signs when available, cognitive function, and chronic disease risk factors using remote monitoring tools when applicable • Provide virtual patient education on preventive measures, lifestyle changes, and community resources • Develop and update personalized prevention plans based on patient needs and physician guidance • Collaborate with the supervising physician on documentation and follow-up for identified health concerns using EHR systems • Identify HCC codes requiring recapture reporting to CMS • Maintain accurate patient records in compliance with HIPAA and organizational standards • Facilitate virtual referrals to healthcare services and specialists • Monitor patient progress and adherence to prevention plans through telehealth follow-ups, providing support and adjustments as needed • Stay current on Medicare guidelines, telehealth best practices, preventive care, and chronic disease management • Participate in virtual team meetings, training, and continuing education

🎯 Requirements

• Fully bilingual in Vietnamese and English • Must be located in Georgia • Current and unrestricted Registered Nurse (RN) license in Georgia • Availability for PRN work Monday through Friday, 8:00am to 5:00pm, approximately 20 hours per week • Minimum 3 years of patient care nursing experience • Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms highly desirable • Knowledge of Medicare guidelines, telehealth practices, and preventive care strategies preferred • Excellent interpersonal and communication skills • Proficiency in electronic health records (EHR) systems, telehealth technology, and remote medical documentation • Ability to work independently and manage time effectively in a remote setting • Strong organizational skills and attention to detail • Internet connection with minimum 100 Mbps download and 10 Mbps upload speeds • Ability to be accessible during stated working hours and participate in virtual meetings with camera on
