📋 Description

• Conduct Medicare Annual Wellness Visits via telehealth under direct physician supervision and in accordance with CMS guidelines • Complete required patient assessments and screenings • Virtually review and update medical histories, medication lists, immunization records, and family histories • Assess current health status, cognitive function, vital signs when available, and chronic disease risk factors using remote monitoring tools when applicable • Educate patients on preventive measures, lifestyle changes, and community resources through virtual consultations • Develop and update personalized prevention plans based on patient needs and physician guidance • Collaborate with the supervising physician on documentation and follow-up for identified health concerns • Identify HCC codes requiring recapture reporting to CMS • Maintain accurate patient records in compliance with HIPAA and organizational standards • Facilitate virtual referrals to healthcare services and specialists • Monitor patient progress and adherence to prevention plans, providing support and adjustments through telehealth follow-ups • Stay current on Medicare guidelines, telehealth best practices, preventive care, and chronic disease management • Participate in virtual team meetings, training, and continuing education • Protect patient and company data by following security policies and procedures

🎯 Requirements

• Fully bilingual in Vietnamese and English • Must be located in Georgia • Current and unrestricted Registered Nurse (RN) license in the state of Georgia • Availability for PRN work Monday through Friday, 8:00am to 5:00pm, approximately 20 hours per week • Minimum of 3 years of patient care nursing experience • Experience in primary care, preventive care, or telehealth preferred • Experience with Medicare Annual Wellness Visits (AWVs) and telehealth platforms highly desirable • Knowledge of Medicare guidelines, telehealth practices, and preventive care strategies preferred • Excellent interpersonal and communication skills • Proficiency in electronic health records (EHR) systems, telehealth technology, and remote medical documentation • Ability to work independently and manage time effectively in a remote setting • Strong organizational skills and attention to detail • Internet connection capable of video calls and internal/external systems • Minimum internet speed of 100 Mbps download and 10 Mbps upload • Must comply with security policies and procedures protecting PHI, PII, and company intellectual property • Must be accessible during stated working hours • Must participate in virtual meetings with cameras on • Must disclose any secondary employment
