📋 Description

• Provide analysis, reporting, education, training, and ongoing support to treating clinicians and care teams to promote accurate and complete documentation of patient conditions • Conduct pre-visit chart reviews and organize relevant clinical information • Identify documentation, coding, and risk-adjustment opportunities for clinician review and clinical consideration • Perform post-visit chart validations and audits • Initiate appropriate clarification queries and provide targeted feedback based on documentation trends • Develop and deliver education on clinical documentation, MEAT, HCC and non-HCC conditions, ICD-10-CM coding, CMS risk-adjustment requirements, and organizational standards • Develop and update presentations, quick-reference sheets, case studies, and other educational resources • Serve as a resource for documentation and coding questions • Facilitate remote and in-person education • Conduct monthly CDI audits and other reviews • Analyze documentation, coding, query, and risk-adjustment performance indicators • Develop individualized education and documentation-improvement plans • Maintain clinician education profiles • Monitor education effectiveness and escalate recurring concerns • Support onboarding and post-onboarding follow-up for newly hired treating clinicians • Collaborate with clinicians, care teams, clinic leadership, coding and billing, and operational teams • Participate in team, coding, interdisciplinary, and other assigned meetings • Travel to assigned clinic locations or organizational events as needed • Protect confidential information and comply with HIPAA, privacy requirements, and organizational policies • Escalate documentation, coding, privacy, or compliance concerns

🎯 Requirements

• Foreign Medical Graduate, or active RN license, as permitted by organizational and state requirements • Certified Risk Adjustment Coder certification or ability to obtain CRC certification within six months of hire • Ability to review medical records, analyze documentation patterns, and provide clear, constructive, and clinically relevant feedback • Strong written, verbal, presentation, facilitation, organizational, and interpersonal skills • Ability to work independently, manage competing priorities, and collaborate effectively across multiple teams • Ability to adapt to changing priorities and processes in a fast-paced environment • Ability to travel as needed • Minimum of two years of experience in risk adjustment, clinical documentation integrity, provider education, medical coding, clinical practice, nursing, or a related healthcare field (nice-to-have) • Certified Professional Coder certification (nice-to-have) • Working knowledge of acute and chronic conditions, clinical terminology, documentation principles, ICD-10-CM coding, and risk-adjustment models, including CMS-HCC V28 (nice-to-have) • Experience providing documentation and coding education to treating clinicians (nice-to-have) • Experience with Medicare Advantage, value-based care, clinical documentation integrity, risk adjustment, or clinician-performance programs (nice-to-have) • Knowledge of documentation and coding requirements for HCC and non-HCC conditions (nice-to-have) • Proficiency with Microsoft Office or equivalent spreadsheet and presentation software (nice-to-have) • Strong clinical reasoning, critical thinking, analytical, and problem-solving skills (nice-to-have) • Experience using athenahealth preferred
