📋 Description
• Process home care referrals and submit clinical data to the home care agencies • Compile clinical information from the EMR to send to the home care agency to facilitate the transition of care • Act as a liaison with the inpatient staff, physician offices and home care agency to ensure the clear communication of the home care orders and the patients’ medical needs
🎯 Requirements
• Associate's Degree in Health Care, Business or related field • 1 year Customer service experience required, preferably in the healthcare industry • Experience entering data, navigating and retrieving information from an electronic medical record (EMR), i.e. Epic, Careport Care Management • Experience in home care placements and or home care intake highly desirable • Knowledge of managed care Medicare, Medicaid or other insurance authorization coupled with understanding of CMS regulations related to homecare services preferred • Excellent verbal and written communication skills • Strong customer service orientation • Ability to adapt to change and work in a team-based environment required • Ability to manage multiple priorities
