• assesses and evaluates members' needs and requirements to achieve and/or maintain optimal wellness state after a hospitalization • guides members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members • employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues • ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations • calls members post discharge to assess needs, review discharge summary and educate on disease progression • maintains relationships with community hospitals and facilities to assist with post discharge planning needs
• Must reside in the state of South Carolina • 3 or more years of clinical acute care experience • Licensed Registered Nurse (RN) in the state of South Carolina with no disciplinary action • Experience with case management, discharge planning, transition of care and/or patient education for adult acute care • Comprehensive knowledge of Microsoft Office applications including Word, Excel, and Outlook
