📋 Description

• Work one-on-one with members to create personalized care plans and coordinate services • Assess members' goals, needs, barriers, resources, and ability to return to the community • Provide specialized support to members receiving Long Term Services and Support (LTSS) • Address health-related social needs and provide psychosocial support • Support members residing in nursing facilities by building relationships with facility staff and advocating for care • Contact members telephonically and face-to-face to establish goals, evaluate resources, develop plans of care, and identify providers and community partnerships • Support members and caregivers in accessing LTSS, social, housing, educational, and other services • Develop and modify Individual Service Plans and involve relevant care team members • Navigate LTSS and related environmental and social needs • Use available member information to prevent duplicative assessments • Recommend Home and Community-Based Services (HCBS) to support member independence • Facilitate interactions with payers, providers, and interdisciplinary care teams • Educate members on maintaining Medicaid eligibility • Enter annual Level of Care assessments into the state portal • Travel 75–90% in the field to visit members

🎯 Requirements

• Must reside in the state of Illinois • Bachelor's degree in social sciences, social work, human services, or a related field • Two (2) years of experience working with Medicare and Medicaid recipients, long-term care services, Home and Community-Based Services (HCBS), or managed care organizations • One (1) or more years of professional experience working with members in custodial and transitional care • Experience using electronic information applications/software programs, including electronic medical records • Ability to travel in the region to meet face-to-face with members, families, community partners, and care teams • Valid state driver's license • Personal vehicle liability insurance meeting required limits • Ability to work Monday–Friday, 8:00 AM–5:00 PM Central Standard Time, with flexibility • Ability to complete TB screening if selected
