📋 Description

• Evaluate members' needs and requirements to achieve or maintain optimal wellness • Guide members and families toward appropriate care and wellbeing resources • Conduct telephonic outreach using an auto-dialer • Assess member needs through data review, structured assessments, and direct conversation • Develop and manage individualized care plans • Coordinate services and connect members with clinical, community, and multidisciplinary resources • Ensure compliance with regulatory requirements • Identify and resolve barriers to effective care and member independence • Monitor progress toward health outcomes and adjust care strategies • Navigate multiple computer systems while engaging members in a fast-paced, structured environment

🎯 Requirements

• Associate degree in any course • Minimum 2 years of experience in direct adult patient care, including long-term care, home and community-based services, hospital/facility setting, case management, or managed care organizations • Experience interacting with members of the healthcare team • Strong computer proficiency • Ability to quickly learn and navigate electronic software programs, including electronic medical records • Exceptional communication and interpersonal skills • Minimum download speed of 25 Mbps and upload speed of 10 Mbps for home internet • Dedicated workspace without ongoing interruptions to protect member PHI/HIPAA information
