📋 Description

• Serve as the clinical lead for technical assistance, training, and practice transformation activities, ensuring the clinical accuracy and quality of all project deliverables. • Provide direct technical assistance, coaching, consultation, and academic detailing to healthcare organizations, clinical leaders, and care teams. • Lead the development and delivery of webinars, Communities of Practice, peer learning initiatives, workshops, office hours, and other clinical learning activities. • Provide expertise on chronic disease prevention and management, care workflow integration, quality improvement, and evidence-based clinical practices. • Advise healthcare organizations on the adoption and integration of digital health technologies, remote patient monitoring solutions, wearable devices, and patient-generated health data into clinical workflows. • Translate clinical guidelines and emerging best practices into practical tools, resources, and implementation strategies. • Collaborate with multidisciplinary teams to design innovative technical assistance approaches that support organizational change and improved patient outcomes. • Support quality assurance, performance monitoring, and continuous improvement efforts across project activities.

🎯 Requirements

• Doctoral degree in medicine, nursing, pharmacy, or a closely related clinical field (MD, DO, DNP, PharmD, or equivalent). • A Master's degree combined with extensive relevant clinical technical assistance experience will also be considered. • Minimum seven (7) years of clinical practice or clinical program experience, including direct experience supporting chronic disease management in primary care, community health, or other healthcare settings. • Minimum five (5) years of experience providing clinical technical assistance, quality improvement support, practice transformation assistance, or provider education to healthcare organizations. • Experience supporting the implementation of technology-enabled care models, including remote monitoring tools, patient-generated health data, wearable technologies, or related digital health solutions. • Demonstrated experience developing and delivering clinical training, technical assistance, coaching, or professional education programs. • Strong communication, facilitation, and stakeholder engagement skills.
