• Provide case management for families of medically complex children enrolled in Imagine Pediatrics’ behavioral health program • Monitor high-risk pediatric patients up to age 21, including recently discharged patients, and ensure appropriate follow-up • Conduct biopsychosocial assessments addressing behavioral, social, emotional, and systemic needs • Create, evaluate, and modify patient and family care plans • Provide interventions for mental health crises, behavioral issues, and family conflict • Facilitate case management and support across systems for mental health diagnoses and co-occurring medical conditions • Resource validated external services such as social services agencies and behavioral specialists • De-escalate and stabilize patients and family members during crises • Provide psychoeducation about mental health diagnoses, treatment adherence, and related information • Collaborate with school systems, specialists, child welfare agencies, and other external care team members • Participate in interdisciplinary consultations to monitor patient progress • Represent Imagine Pediatrics to patients, families, providers, and the community • Perform other duties assigned by the manager
• Master’s degree with major coursework in social work or a related field required • LMSW required • Minimum 3–5 years of postgraduate experience in healthcare social work/case management in behavioral health required • Experience working with pediatric populations and family systems required • Proficiency in motivational interviewing, goal setting, intervention, and needs assessment • Knowledge of social work, including crisis prevention and intervention • Experience providing telehealth services • Knowledge of MS Office Suite and ability to work in online platforms • Strong knowledge of behavioral health principles and practices • Proficiency in trauma-informed care practices • Strong knowledge of common signs and symptoms of pediatric/adolescent mental health conditions and ability to identify coping difficulties • Remote role with 10% travel required for training and education • Ability to work afternoons and evenings • Bilingual Spanish strongly preferred
