• Support participants in their healthcare journey and help them achieve measurable health behavior change • Guide individuals in setting and reaching wellness goals, including lifestyle modification and chronic condition prevention and management • Review pertinent medical history and participant information • Perform and/or record anthropometrics as applicable to care recommendations • Assess participant needs, readiness for change, and self-efficacy using evidence-based coaching models • Work as part of an interdisciplinary team to support changing health behaviors • Engage participants through individualized strategies and interventions considering culture, language, age, and gender • Provide education within clinical scope using adaptive coaching strategies • Conduct 30–60 minute live or digital coaching sessions • Document participant activities and coaching/counseling sessions • Demonstrate customer service behaviors and treat clients with courtesy, dignity, respect, and professionalism
🎯 Requirements
• Must have completed a health coaching program approved by the National Board for Health and Wellness Coaching (NBHWC) certification prior to hire • Current Health and Wellness Coach Certified or will obtain National Board for Health and Wellness Coaching (NBHWC) certification within 12 months of hire • Demonstrated experience in a healthcare or applicable care setting • Knowledge of motivational interviewing • Knowledge of adaptive coaching strategies • Disease prevention coaching knowledge • General wellness knowledge • Navigation of medical settings • Strong written, verbal, and digital communication skills • Microsoft Suite proficiency, including MS Word, Excel, and PowerPoint • Time management and organizational skills • Ability to perform the stated physical requirements, including frequent computer use and typing
