IMH also runs digital platforms to support project estimation and paid home-service offerings, coordinates large-scale catastrophe responses, and connects insurers, beneficiaries and service providers via its accredited prestataire network.

📋 Description

• Perform psychosocial assessments • Develop and implement care plans with the appropriate care team • Assess crisis situations and provide clinical counseling, diagnosis, brief therapeutic interventions, and resources or referrals • Provide individual and family treatment as indicated • Evaluate patients for transition planning, mental health, substance use, and goals of care • Develop and monitor care plans addressing social determinants of health and community resources • Use motivational interviewing and therapeutic techniques for mental health care, behavioral change, trauma-informed care, and substance use disorders • Provide brief individual, group, and family therapy • Identify mental, emotional, and behavioral disorders and connect patients to services • Train staff, educate patients, and advocate for rights and care access • Facilitate safe transitions of care to the community • Collaborate with patients, support persons, healthcare providers, insurers, community resources, and other involved parties • Lead improvement initiatives, track key metrics, and ensure policy adherence • Facilitate clinical goals-of-care discussions with patients, families, and teams

🎯 Requirements

• Master of Social Work (MSW) from an accredited institution; degree verification required • Current state licensure, as applicable, obtained prior to or upon completion of required supervision hours (CSW/LCSW) • Basic computer proficiency, including familiarity with word processing and spreadsheet software • Strong written and verbal communication skills • Demonstrated ability to apply critical thinking skills • For duties requiring home or community visits: current BLS certification, current driver’s license, current auto insurance, acceptable driving record, and reliable transportation • Preferred: Case Management Certification • Preferred: Experience in clinical care management, social work, or working with third-party payers • Preferred: Understanding of care management principles and practices • Preferred: Understanding of health insurance products and related processes • Preferred: Understanding of coding, episode of care, and length of stay guidelines • Ability to work independently, demonstrate self-motivation, maintain a positive attitude, and adapt to a rapidly changing environment • Ability to see and read information, documents, and monitors and assess customer needs • Ability to verbally communicate and hear and understand spoken information accurately • Manual dexterity to manipulate complex and delicate equipment; frequent computer use • For driving roles: ability to drive and read signs, traffic signals, and other vehicles
