📋 Description

• Improve the overall quality and completeness of clinical documentation • Facilitate modifications to clinical documentation through concurrent interaction with physicians and coding staff • Ensure records accurately reflect diagnoses treated, procedures performed, and patient severity of illness • Communicate with physicians about missing, unclear, or conflicting documentation through the query process • Obtain needed documentation, present opportunities, and educate physicians on appropriate identification of severity of illness • Collaborate with physicians, case managers, coders, and healthcare team members to facilitate comprehensive inpatient medical record documentation • Apply knowledge of complications, co-morbidities, severity of illness, risk of mortality, case mix, secondary diagnoses, and procedure impact on the final DRG • Identify patterns, trends, variances, and opportunities to improve the documentation review process

🎯 Requirements

• Associate's Degree in Nursing or Healthcare related field — Required • Bachelor's Degree in Nursing or Healthcare related field — Preferred • 7 years acute care nursing experience in lieu of Bachelor's Degree — Required • 2 years recent inpatient, case management, or ED experience — Required • Registered Nurse (RN) — State Licensing Board — Preferred • Certified Clinical Documentation Specialist (CCDS) — ACDIS — Preferred • Travel is required never or rarely
