📋 Description

• Collaborate with the team and assist in coordinating care for mental health, behavioral, and social needs • Facilitate communication among patients, families, healthcare professionals, community and state agencies, and interdisciplinary team members • Assist with implementation of care plans and coordination of behavioral health and social services • Provide evaluation and treatment support for psychiatric patients, including psychological assessment, crisis intervention, case management, patient advocacy, care coordination, and community outreach • Provide input in treatment decisions and present treatment plans for approval • Obtain and document patient information in the medical record • Involve patients, families, and others in care delivery and treatment recommendations • Work with patients to determine treatment goals • Arrange safe and expedient patient transfers • Serve as an information resource for division members, patients, and referral sources • Assist with provider schedule maintenance • Set up appointments and arrange follow-up treatment • Assist with insurance verification and authorization procedures • Support interdisciplinary care from the acute episode through post-discharge • Liaise between the hospital and community agencies or facilities for clinical and referral information exchange • Identify high-risk patients from psychosocial and financial perspectives • Assess psychological needs and provide information, support, counseling, care management, and referrals • Resolve psychosocial patient-care issues • Develop and implement complex patient transition and discharge plans

• Bachelor's Degree-Social Sciences (Required) • Experience: Not specified • Skills: Patients Rights, Teamwork • Must comply with all organization policies and procedures • Does not qualify for J-1 waiver • Work is typically performed in a clinical environment
