• Act as a liaison between clinical and coding functions • Educate physicians, medical staff, and clinical professionals on clinical documentation improvement and Revenue Management processes • Review inpatient medical records within 24–48 hours of admission • Evaluate documentation to support principal diagnosis, secondary diagnoses, procedures, DRG assignment, severity of illness, and risk of mortality • Conduct subsequent record reviews every three days through discharge • Formulate queries for missing, conflicting, or unclear documentation • Interact daily with physicians and clinical professionals to clarify and optimize documentation • Collaborate with nursing staff, nutritionists, pharmacists, physicians, and inpatient coding professionals • Resolve documentation queries before patient discharge • Meet established productivity targets for record review • Identify strategies for sustained work-process changes supporting complete and accurate documentation • Analyze and trend statistical data for the assigned patient population and identify improvement opportunities • Promote accurate principal diagnosis, procedure, supporting documentation, working DRG, and final DRG assignment • Serve as an interdisciplinary resource to promote coordination of patient care while considering age-specific, developmental, cultural, and spiritual needs
• At least three years of RN/nursing work experience • Graduate from Specialty Training Program-Nursing required • Active Licensed Registered Nurse license in Pennsylvania required • Understanding of anatomy and physiology • Clinical knowledge of underlying etiology, principal diagnosis, diagnostic studies, and treatment modalities • Experience or proficiency with medical records systems • Communication skills • Multitasking ability • Ability to review inpatient medical records and support accurate DRG assignment, severity of illness, and risk of mortality • BSN preferred • Full-time availability for 40 scheduled hours per week
