📋 Description

• Deliver fully remote, video-based, comprehensive clinical care • Provide preventive care, sick visits, and chronic condition management • Provide high-quality preventive, acute, and chronic care in a virtual setting • Elevate remote care through compassionate and meaningful patient encounters • Manage a dedicated panel of patients as primary care physician • Refer patients to specialists or acute care services when clinically indicated • Work on population health initiatives for risk-attributed patients, including AWVs, care gaps, transitions of care, and risk capture • Support patients immediately post-discharge to improve treatment-plan understanding and reduce hospital readmissions • Practice continuous self-evaluation to ensure adherence to clinical guidelines and best practices • Provide overnight coverage to two partnership clinics through 10-hour overnight shifts • Practice the full spectrum of primary care, including pediatrics and mental health • Collaborate with Care Concierge, Clinical Operations, and technical support teams

🎯 Requirements

• Board-Certified in Family Medicine or Internal Medicine • 1+ year of post-residency work experience in an in-person primary care setting • Experience with virtual visits required • Must have prior experience with EPIC EMR • Clean background and medical malpractice history • Willing to work a rotating schedule, 7 days on, 14 days off • Spanish-speaking is a plus • Strong interest in developing longitudinal relationships with patients and managing chronic care conditions • Comfort with management of acute conditions in a virtual setting • Extensive experience handling 3+ cases per hour • Eagerness to learn about and implement cutting-edge technology to improve efficiency and clinical outcomes • Licensed in NY, WI, MN, IA, or willing to obtain these licenses • Board certification in Family Medicine or Internal Medicine
