• Ensure proper collection, labeling, storage, and transport of samples • Follow CLIA guidelines to identify patients using unique identifiers • Verify all test requisitions are properly filled out • Obtain proper samples via venipuncture and/or fingerstick • Label all samples with unique identifiers • Maintain documentation and identification of samples for transport • Perform blood culture collection without contamination • Explain proper collection protocols for various samples • Observe safety protocols and use PPE • Follow Universal Precautions and OSHA Standards • Maintain daily tallies of collections performed • Inform Phlebotomy Supervisor of significant shifts in collection volumes • Request supplies as needed for daily duties
• High School Diploma, GED, or equivalent • Phlebotomy Certificate (proof of certification required upon hire) • Computer and technology proficient • Must maintain a current driver’s license and clean driving record
