📋 Description

• Provide evidence-based virtual primary care for patients aged 18 and older • Perform accurate, complete, concise, relevant, and age-appropriate history and physical assessments • Evaluate and treat patients’ medical needs with safe, high-quality care • Counsel and educate patients and families • Document patient care in the electronic health record according to current policies and procedures • Collaborate with the dedicated care team and internal and external colleagues to coordinate patient care • Provide compassionate, appropriate, and effective patient-centered care • Promote health, prevent illness, and treat disease • Refer patients to appropriate specialists and care components • Apply quality measurement awareness in chronic disease management • Document patients wishing to join the care team panel using approved EMR workflows • Participate in or lead practice committees such as Epic Optimization and Care Pathways

🎯 Requirements

• Active Advance Practice Registered Nurse, Nurse Practitioner license in good standing • Current National Board Certification, Family Practice • 5+ years of recent Primary Care experience as a Nurse Practitioner in family medicine or primary care • Effective verbal, written, and electronic communication skills • Outstanding organizational skills and ability to multi-task • Initiative, problem solving ability, adaptability and flexibility • Ability to work remotely in a care team culture • Proficient with information management and technology • Obtain and maintain in good standing, multi-state licensures • Willingness and ability to take after-hours and/or overnight call to support patient care • Completion of master’s degree level Family Nurse Practitioner program • Current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role • Broadband connectivity, quiet setting with neutral background, and ability to uphold patient privacy per CVSH guidelines • Ability to interact remotely with patients, sit, write, operate computer and phone, speak intelligibly, and hear patient responses • Ability to view and read a computer screen and other electronic devices
