📋 Description

• Coordinate admission, transfer, and consultation of emergent, urgent, and elective patients into accepting facilities • Process patient acuity and make clinical decisions to facilitate patient flow through the continuum of care • Document and prioritize patient information for accepting physicians, nurses, and healthcare workers • Maintain knowledge of departmental procedures, protocols, and communications • Assign transfer priority levels for triage and transportation • Maintain Transfer Center data collection and provide feedback on patient transitions and outcomes • Screen admissions for appropriate level of care, status, and bed type • Apply Medical Necessity criteria • Educate nurses, physicians, and ancillary staff on level-of-care and admission/transfer issues • Act as a liaison among physicians, patients, families, hospital staff, outside agencies, and patient-care areas • Monitor compliance with regulatory requirements, EMTALA, ethics and compliance policies, and quality initiatives • Track and trend barriers to patient flow; recommend improvements and develop action plans • Escalate Transfer Center issues to leadership and facility administrative leadership • Coordinate with the Bed/Capacity Coordinator for timely transfers and accurate bed assignments • Maintain accurate information in the Electronic Health Care Record • Participate in orientation, ongoing training, staff meetings, and continuous quality improvement projects • Handle inbound and outbound calls to meet productivity goals • Provide superior customer service and support Transfer Center performance indicators

🎯 Requirements

• Graduate of an accredited school/college and holds applicable current RN licensure in state of residence • Current nursing licensure in State of Residence; Preferred Compact License • Must maintain current nursing licensure and complete continuing education requirements • Three years of related clinical experience in an acute care setting (ED or Critical Care preferred) • Ability to communicate effectively in oral, written, and electronic formats • Ability to attentively listen and simultaneously type more than 35 wpm • Internet speed requirements: Upload 50 or more; Download 100 or more
