📋 Description

• Administer, monitor, and discontinue intravenous therapies in patient homes or infusion suite settings • Insert and manage IV access including peripheral lines, PICC lines, ports, and midlines • Monitor patients for adverse reactions and respond appropriately to changes in condition • Perform venipuncture and collect blood samples as required • Complete patient assessments, vital signs, and reviews of systems during each visit • Provide individualized patient and caregiver education on infusion therapy, line care, and symptom monitoring • Coordinate and communicate visit details, scheduling updates, and clinical documentation with internal care teams and external partners • Document all patient-related activities accurately, including medications administered and patient responses • Assist with planning, organizing, and coordinating patient care delivery across nursing personnel • Communicate concerns or reportable events promptly to physicians and internal clinical leadership • Uphold all confidentiality, compliance, and ethical standards related to patient care

🎯 Requirements

• Nursing degree from an accredited college or university • Current, unrestricted Registered Nurse (RN) license in the state of practice • Minimum of two (2) years of RN experience • IV certification and experience with central line care and management • Valid driver’s license and ability to travel to patient homes • CPR certification • Experience with patient assessments, documentation, and care coordination • Basic computer proficiency (Microsoft Office and clinical systems) • Willingness to travel throughout assigned service areas
