📋 Description

• Coordinates and supports mental health care to patients enrolled in Collaborative Care Management (CoCM). • Provides evidence based, short term interventions, case management needs, and acts as a core member of the CoCM team. • Screens and assesses patients for common mental health needs, monitoring treatment efficacy. • Communicates recommendations to Primary Care Providers. • Tracks/document patient follow-up and clinical outcomes in an EPIC based registry. • Completes relapse prevention plans with patients in remission.

🎯 Requirements

• Bachelor’s Degree in social work, psychology, counseling, nursing or other related mental health field. • 2 years of experience in mental health/behavioral health experience or related setting. • Formal education or specialized training in behavioral health. • Demonstrated knowledge and experience in a behavioral health field. • Ability to work effectively with people from a broad range of backgrounds. • Must possess a high degree of emotional intelligence. • Knowledge of evidence-based interventions such as Motivational Interviewing, Problem Solving Techniques, and/or Behavioral Activation.
