📋 Description

• Provide administrative support to the Care Management team • Collect data for Health Risk Assessments (HRA) • Screen members for eligibility, benefits, and Care Management service priority • Identify members without a PCP and refer them to Member Services • Perform transition-of-care duties and contact PCPs, medical POAs, and other providers • Document encounters and contacts, complete monthly reports, and maintain electronic member records • Motivate members to participate actively in their health and well-being • Provide health education, member follow-up, arrange PCP visits, and coordinate care under the Care Manager's direction • Help members access medical homes, health-related services, and social services • Verify appointments and obtain medical records to promote member compliance • Connect members with community resources including housing, food, clothing, prenatal classes, parenting resources, life-skills providers, and mental health services • Establish positive relationships with members, providers, nurse care managers, social workers, agency representatives, and office staff

🎯 Requirements

• High school diploma or equivalent • 2+ years serving the Medicaid population, preferably in the MD/DC area • 2+ years OB/GYN experience • Knowledge of medical terminology • Strong interpersonal, communication, and customer service skills • Ability to work effectively with a wide range of constituencies in a diverse community • Knowledge of community agencies and resources • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services • Knowledge of transportation and other barriers to care • Ability to work collaboratively and effectively within a team
