📋 Description

• Conduct virtual initial assessments and ongoing care for seniors with serious mental illness (SMI) and substance use disorders (SUD), in collaboration with psychiatrists and other healthcare professionals. • Develop comprehensive treatment plans, which may include coordinating psychotherapy or other treatment modalities, and assessing the need for referrals to specialized care. • Collaborate effectively as part of a multidisciplinary team to address the holistic needs of patients, including their physical health, behavioral health, and social well-being. • Provide patient care and education tailored to the individual’s age, abilities, educational background, and cultural context, often involving family members in the care process. • Coordinate care with primary care physicians (PCPs) and other medical professionals to ensure a cohesive approach to patient health. • Offer timely psychiatric urgent care and be responsive to patients’ immediate needs. • Document all clinical work comprehensively and promptly in the electronic health record (EHR).

🎯 Requirements

• Current LCSW licensure in Florida with eligibility for Texas licensure. • 3-5 years of experience post-licensure. • Bilingual proficiency in Spanish and English (preferred) • Comfortable navigating the dynamic landscape of a startup environment. • Proven ability to work collaboratively within a multidisciplinary team. • Commitment to maintaining professional boundaries and integrity in patient-therapist relationships. • Skilled in engaging patients and caregivers through telehealth and telephonic communication. • Demonstrate proficiency in electronic health record (EHR) systems for accurate and timely documentation.
