📋 Description

• Lead clinical strategy at scale for post-acute care • Guide the design, launch, and expansion of specialty consult services across multiple states • Work with physician specialists, executive leaders, NPs, PAs, and facility teams • Define clinical protocols • Oversee quality metrics • Mentor providers and develop advanced practice providers • Manage multi-site clinical operations • Monitor dashboards • Ensure CMS and state compliance • Lead root-cause analyses when needed • Align specialty strategy, quality, workforce development, and operational execution • Support consultative services in cardiology, pulmonology, nephrology, and other specialties

🎯 Requirements

• Master of Science in Nursing (MSN) • Active Nurse Practitioner license in good standing (multi-state preferred) • 10+ years of NP experience in PALTC, SNF, or LTACH settings • 5+ years of experience in leadership, education, or program development • Clinical background in cardiology, pulmonology, nephrology, or infectious disease • Proven success building and leading NPP teams • Strong knowledge of CMS regulations, documentation standards, and value-based care • Proficiency with EHR systems, dashboards, and care coordination platforms • Ability to operate at scale while empowering clinicians on the ground • Doctor of Nursing Practice (DNP) strongly preferred
