âą Perform remote monitoring and principal care management for patients under the supervision of the medical team âą Track and monitor patient data and communication across members of the medical team âą Engage with patients and family members âą Report clinical summaries âą Assist clinicians with gathering medical information between visits using electronic health record systems âą Report monthly medical concerns, including falls, medication issues, behavioral concerns, and recent hospitalizations âą Complete assigned PCM interventions each month for approximately 15 patients per day âą Report issues to the ancillary program manager and/or Chief Clinical Officer when applicable âą Assist with employee trainings and support newer employees through shadowing
âą Candidates must live in Mississippi, Alabama, Ohio, Iowa, Missouri, Pennsylvania, or West Virginia âą Must be able to work EST hours âą Minimum high school diploma required âą Quiet space free of distractions with internet at home for remote work âą High organizational skills and proven follow-up ability âą Basic knowledge of medical terminology âą Attention to detail âą Excellent written and verbal communication skills âą Thorough understanding of healthcare technologies, including Electronic Health Record Systems, Microsoft Outlook, Microsoft Excel, and internet/cloud-based applications âą Ability to communicate effectively and professionally with facility staff, colleagues, and medical team members
