📋 Description

• Participate as part of the Health Care Team to facilitate quality patient care • Integrate and coordinate service delivery, care facilitation, payer authorizations, service access, discharge, post-discharge activities and utilization review • Review patient orders, verify insurance information, gather clinical data and determine patient care needs • Identify patients meeting criteria for Alliance Services • Facilitate transfers from hospital to home care/hospice services • Consult with nursing, social services and other support services • Build relationships with referring physicians and hospital Case Management staff • Provide information and education to health care teams, physicians, hospital staff and patients/families • Answer questions and requests and complete required reporting • Communicate orally and in writing with patients, families and health team members • Deliver and set up portable medical and related equipment at assigned facilities • Ensure equipment operates safely and that proper training is completed • Demonstrate and promote professional nursing practice

🎯 Requirements

• Required Associate's Degree in Nursing • Registered Nurse with 3–5 years of experience • Required Registered Nurse licensure through the Tennessee Board of Nursing • Required Basic Life Support certification from the American Heart Association • Preferred Bachelor's Degree in Nursing • Preferred home care/hospice experience • Preferred certification in a clinical specialty • Patient advocacy, communication and interpersonal skills • Nursing principles, concepts and techniques • Analytical skills for assessing complex patient conditions and developing comprehensive care plans • Ability to develop and maintain working relationships with physicians and health professionals • Facilitation, planning, scheduling, independent judgment and critical-thinking skills • General computer skills required; advanced computer skills preferred • Knowledge of supplies and related equipment • Ability to handle and administer narcotics as needed • Ability to react quickly to emergency situations • Ability to perform medium work, exerting up to 50 lbs. occasionally and/or up to 25 lbs. frequently • Frequent invasive and non-invasive patient contact and exposure to body fluids and hazardous or poisonous materials
